INFORMATION ON THE PROCESSING OF PERSONAL DATA

Dear Sir or Madam,
Fulfilling the obligation arising from Art. 13 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), you need to be advised what follows:

The Controller of your personal data is:

  3.  The legal grounds for the processing of your personal data are the provisions of the General Regulation on the Protection of Personal Data of April 27, 2016 (GDPR).
  4. Your personal data is processed for the  purpose of:
    a) concluding and performing of the contract – Art. 6 Section 1 letter b) GDPR,
    b) processing is necessary for compliance with a legal obligation to which the controller is subject – Art. 6 Section 1 letter c) GDPR,
    c) processing is  necessary  for  the  purposes  of  the  legitimate interests  pursued by  the  controller – Art. 6 Section 1 letter f) GDPR.
  5.  The recipients of your personal data are institutions and companies providing services to the Data Controller (accounting, IT, legal, forwarding, other), state authorities.
  6.  Pursuant to the provisions of the GDPR, you have the right to request from the Data Controller access to the content of your personal data, rectification, erasure, restriction of processing, objecting to processing, portability and to withdraw the consent at any time without affecting the compliance with the law of processing, which was carried out on the basis of consent before its withdrawal.
  7.  You have the right to lodge a complaint with the supervisory office – the President of the Office for Personal Data Protection.
  8.  Your personal data will be processed until the end of cooperation, and after that for the period of asserting possible claims between the parties under the contract or applicable law.
  9.  Your personal data will be stored for the period as required to comply with the provisions on archiving personal data applicable for the Data Controller.
  10.  Your personal data shall not be processed in an automated manner, including in the form of profiling.
  11.  Your personal data shall not be transferred to a third country within the meaning of the provisions of the GDPR.
  12.  Providing personal data is voluntary, however, refusal to provide such data may result in refusal to conclude the contract.
